Community Health Partners (CHP) relies on an understanding of the social determinants of health, a patient- and staff-centered culture, and use of transparently shared data to drive staff engagement in quality improvement work, aligned with the organization’s mission, vision, and values.  CHP was recently awarded Level 3 Patient-Centered Medical Home recognition from the National Committee on Quality Assurance.

Why this toolkit?  Community Health Partners (CHP) is a Federally Qualified Community Health Center providing high-quality, affordable medical, dental, behavioral health, pharmacy, and educational services to over 12,000 clients annually in rural and frontier Park and Gallatin Counties of Southwest Montana. Over 95% of CHP patients reporting incomes earn less than 200% of the Federal Poverty Level annual income, and therefore one of CHP’s six strategic goals is to address root causes of poor health such as income, education, housing, transportation, and access to healthy food in pursuit of our mission to enhance community health and well-being.

This toolkit is a culmination of years of experience developing educational programming integrated into the health center environment. Our hope is that this toolkit, combined with stories of creative implementation and programming, and research showing the effectiveness of early childhood intervention on long term health and well-being outcomes, you will be armed with the tools you need to begin addressing root causes of poor health at your clinic or health center.
